Preparing for your appointment with Dr. McMorrow is key to getting the most from the visit. Gathering your information prior to the visit will improve your patient experience. The staff will assist you through this process. We need you to tell us about your medical history and prior treatment. Neurosurgery of Western NY has appointment forms available on this website. Please complete paperwork prior to appointment. Otherwise, please arrive early to complete these forms. All paperwork must be completed prior to being seen. Dr. McMorrow and her assistant need to review this information to provide comprehensive care.

If you’ve already scheduled and/or registered for our Patient Portal, here are some things to make sure to bring to your first appointment:

  • Photo identification (Driver’s License or State-issued ID)
  • Any applicable health insurance ID cards or coverage statements
  • A detailed list of any current medications (prescriptions, over the counter, and dietary supplements)
  • Any copayment required as part of your insurance plan
  • Past medical records, imaging CD’s, etc.
